Question in I-134.....plz help

Hi,

We have booked 2 diffrent interview dates for my parents .they are going on Sep 26(mother) & 27(father).I have 5 questions:

1) Shud we send 2 seperate I-134 on seperate names for my parents or one on my fathers's name is sufficient?

2) Will the I-134 be asked for submission or will they return it to my mother?

3) shud i mention my mother's name for question 9 in my father's I-134?

4) regarding the bank statements...shud we send the originals or the copies and how many is really necessary?.

5) shud i notarize any other form other than I-134?

Yes u need 2 (one for each) since they are going on different days

Yes, they will take it

Theoretically, yes

Better send the originals. last 3 months will be enough

5) shud i notarize any other form other than I-134?
