Property estimate question

Hi Guys,

Do we need a property estimate given by a CA in addition to the notarized affidavit that outlines what the value of the property is? If so, is there any particular format for the document to be provided by the CA?

For my in-laws, I took property estimate by a CA but not notarized affidavit.
Generally CAs will have format
