DS-156 form questions

  • DS-156 form questions

    Hi Guys,

    Can some clarify the following for me with respect to filling in the DS-156 form?

    1) Question 16: We just have a home telephone number. If I attempt to enter "NA" for all other phone numbers, I'm unable to submit the form. So I have to leave it at (91) followed by blank. However when I print the form, the phone number fields are all blank. Can I manually write "NA" on the printed form? Wouldn't that be different from the consulate sees i.e they just see empty boxes?

    2) Question 20: Name & address of present employer or school. These fields automatically got prefilled to be "NA" based on the response "No" to the question "Are you studying or working"? while filling the form. However when the form is printed out, 2 blocks for Question 20 are just empty. So can I manually write "NA" on the printed form? Wouldn't that be different from the consulate sees i.e they just see empty boxes?

    3) Question 29: I'm pretty sure when filling the DS-156 form online, I chose option "Yes" to the question "Have you ever been in the US". However when I print the form out, both "Yes" & "No" are no checked , but the information entered regarding the entry is shown. So can I manually check the "Yes" box on the printed form? Wouldn't that be different from the consulate sees i.e they just see unchecked boxes?

    4) Question 37: I'm currently on H1 extension (parallely on I-485 stage in my GC). So do I mention "H1B worker" or just say "working" according to the legend on the form for this question

    Appreciate if you guys can clarify the above questions

    Dont write any thing, blank is okay,

    Edit, save and print one more time, you should be good, if still did not show, highlight the correction and explain when asked.
