Surname Help

Hi, Everyone

i am having trouble interpreting passport names and how to map them
to ds156 form. I have already filled out the form online and sent a copy of this to my parents already. Please help

Passport Details -
Surname (Sur is scratched out) - Ramnathan
Given Name - Gowda

This is how i have filled ds156
6. Surname(As in Passport) = Ramnathan
7. First and Middle Names = Gowda

Is this correct??

Given name in passport maps to First name and Surname maps to Last Name

But if you notice the Sur in Surname has been scratched out, what do i do?
