help in filling DS156

Hello,

I need clarification for filling form DS156 point No 8 and 9 for My mother -- Other Surnames and Other First and Middle Names used

My mother has issued passport in her new name after marriage. Her School Leaving Certificate has her old middle name and old surname.

So do i need to mention her old middle name and old surname in point No 8 and 9. She is not having Marriage certificate also.

Does LC needs to be shown at the time of interview?

If you specify her other first name and last name as per Leaving Ceritificate then your mother needs to carry School Leaving certificate for the interview.
Otherwise it is okay to specify no, (if it is not there in her passport)
