Time frame for first entry to US on Visitor visa

hi everyone.
My mother got a 10yr multientry visa on May 2006, But she couldnt travel immediatly due to some personal reasons. Now its been almost 5 months, Is there a time period in which the first entry to US should be made after the visa is issued? if so what is it. And if she crossed the time limit, what is the proceedure to get this extended.
