Adding the Last Name

My father applied for a passport and in the application he left the
last name ( surname ) field empty. The passport was issued to him
and in the Given Name he had his entire name "Madhavan" and the
surname was empty.

He went for his visa interview last year and got the US Visa.
In the visa the First Name was "FNU" and last name was "Madhavan".
He came to visit me in jan 2006. We travelled in and around US and
did not face any problems. Now at the time of going back to India,
we were told at the ticketing counter that if he does not have his
last name in his passport the airline would not allow him to board
the flight. Then after a lot of persuasion and talking to the higher
authorities he was allowed to and we were asked to change the passport
and include the last name.

He is in India now and we want to change the last name. When contacted
the passport office, they said we would be issued a new passport with
the changed name. So the changed passport would read something like
Surname : Iyengar
Given Name : Madhavan
( holding off to clarify this before applying for the new passport )

The visa has his last name as "Madhavan" and first name as "FNU".
With the changed passport do we have to change the visa as well. If
it is needed to change the visa, is it like getting a new appointment
and going through all the steps etc. Anyone faced this problem kindly
let me know as to how to proceed.

You will have to go through everything all over again to get new visa. There is no such thing as getting visa changed.
