    Hi

    Is it necessary to send two I-134 forms for parents?

    It is not necessary. But lot of people
    recommend to take separate I-134
    for each person. Why to take
    chances ? Its just $5 per one notary
    in Bay area

      Yes. Each applicant needs to have a I-134 filled out. That would be one for you father and one for you mother.
