My company has group life insurance for me and I also purchased some supplemental life insurance. Should I put total amount here and what's the cash surrender value?

Group life insurance through employer is always term life insurance and cash surrender value is zero.

For the insurance you purchased, what type of insurance is it? If it is term life insurance, it is again 0.
If it is whole life insurance or universal life insurance, ask your insurance agent or insurance company for the cash value.
