Regarding Property Estimate Affidavit-for B2

I have applied for a B2 visa, Can anyone provide me, format for the property estimate Affidavit to be given by the CA ?

As I am taking bank statements seperatly, do I need to mention about the bank-balance in the property Affidavit ?
