Gurus please help answer following of I-134 form:

2. That I am .......and have resided in the USA since??
The answer should be last time I went to USA or the first time that I went to USA?

9. Previously submitted affidavits of support for following persons....
What should be the answer. Is this going to be last time I have submitted I-134 for parents even though their VISA was rejected.
