if owns business...how should I write in ds-156 20 questions...if there is no partener ship..

because in sample 156 form for murty, is showing that even though he is business owner he filled 'present employer/school' name ..?

If the person is business owner or self-employed, the employer name should be the name of his business.

in 18 question 'spouses' full name include maiden name' means...
for wife name we have to write as in passport or include mainden name before marriage?

in sample letter they mentioned surname same as her husband..didn't changed..
