Box 18 Spouse name great confusion!!

Hello All,
This is my second attemp at clearing confusion regarding filling information in Box 18 of DS 156. My question is -Are we required to write both the current name of spouse(wife) and her maiden name(name before marriage) also??. For example Shreya Bharat Shah (current name) / Shreya Vinod Shah (maiden name). The help team guided me last time to the form filling help section, but again there is not much clarity over there. Please someone can clarify??? Many readers would benifit from this.

You can write like this in 2 lines:

Shreya Bharat Shah (current)
Shreya Vinod Shah (maiden)

Or you can just write
Shreya Viond Shah

In US, woman's original name before marriage more important than the changed name after marriage because many people keep marrying and divorcing several times. However, original name remains same.
