Edit in first name/last name in DS-156 .... URGENT!

My mother's visa interview is on Nov 30 in Chennai consulate. When I had applied for her interview slot via VFS some time back, I had done that using her name in her passport. Later, an observation was done in her passport for her name. This observation was to interchange her first name and last name.

For eg. Her name earlier in the passport and while applying in VFS was First name: X, Last name: Y
The observation says that her first name is Y and last name is X.

Since I was not able to edit her name in the DS-156 online, I downloaded it and then using a pen interchanged her first name and last name after striking off what was wrong. I put my initials against it. I also asked my mother to put her initials next to this change.

I am now concerned if this might be a reason for rejection. If yes, then I'd like to know what needs to be done for this.

Did your mom face any problems for over writing on the form (i hope not) coz I need to put in my moms maiden name which i forgot?

I guess you are not supposed to submit any handwritten/corrected form. So my mother gave the DS-156 as is, without any change (she was also carrying the corrected form as a back up, just in case). She did not face any problem. She was granted a visa for 6 months. Still waiting for the passport to arrive to find out if it is for 10-yr multiple entry.
