Question - Visitor Visa validity

Hi,
My in-laws got 10 years multiple entry visa in September 2006. Is there a time limit within which they have to intiate their first trip to USA?

Is there any recommendation for health insurance?

Congratulations! There is no time limit. They can visit anytime in the next 10 years.

I am considering buying Reliance or Oriental Insurance from India.
