Doubt in DS-156 Q18

In the DS-156 ..Q18 they have asked to enter Spouse's full name (Include even if divorced or separated). I just wanted to know if we need to include Spouse's maiden name in all cases or only in cases where they are divorced or separated?

My mother's maiden name is not there on her passport anywhere?

they dont want it because it is on tha passport but her name before marriage was part of her identity thats why
