Help needed on Q8&9 on DS-156

Hi,

I have a quick question on what to fill in Q8 and Q9 on DS-156. If there is no maiden name in the passport do we still need to mention the maiden name in the form? My mom's maiden name is not mentioned in her passport anywhere and ofcourse there is no documentation showing her maiden name...do we still need to fill the maiden name in the form?

You don't really have to.

However, what is the name shown in her birth certificate or records before marriage such as school leaving certificate? You can enter that name.

I don't know if my mom has a birth certificate till date and i'm not sure abt the school certificates....does she need to show all these docs?

If there are absolutely no records of her maiden name, you don't have to worry about it.
