About other name on DS-156

HI,

I have question about Q.No 9 on DS156. My fathers name in passport is 'Krishnamoorthy' , so I will be entering as it is for Q.NO 7. His all other documents like House documents , business documents and bank documents is 'Krishna **** **'.

Do you think shoud I mention' Krishna **** **' in Q.NO 9 (other first middle Names used).
