Ds-157 Urgent Please

My Mother is 1950 born and does she require DS-157 form
because when i booked the application through VFS, it just gave me only DS-156 and also in this site, it says that DS-157 is required for 15-55 years of age
please advise as early as possible

DS157 is required only for below 55yrs of age. My dad is 60, he didnt fill DS157. Got visa today.
