Name of Visitor on I-134

Hi,

A quick question on filling I-134 Affidivit of Support. My mom's last name is blank in her passport and her entire name is written in Given Names section. How do I write the name in the Affidivit of Support that I am sending? Should it be as per her passport ?

Let's say your mother's name is as follows:

First Name: Geeta
Last Name: Sharma

And it is written as Geeta Sharma in the Given Names section.

You should just write her name as 'Geeta Sharma' everywhere.

However, please note that when her visa is issued, they will issue as follows:

Firstname: FNU
Last Name: Geeta Sharma

If possible, get her passport changed to reflect correct name if you have time.

Thanks for the reply. I do not have time to get anything changed in the passport.

I was planning on putting under her first name: Her first name
Under middle name: her middle name and last name.

This is I-134.
