How to download Original Application form?

I have applied for my in-laws visa in Sept 06. Appointment is on 21st March at Mumbai Consulate. As a part of requirement i am trying to download original application forms ( DS-156/157) from VFS website.

I am getting "Error" without any specific details about error. Please someone tell me how to download these application forms. Original application was submitted to VFS center in Pune. Can i visit VFS center in Pune to get these forms?

Original Application DS-156 / DS-157 were typed and printed.
