65 years Old without DS-157

My mom has been refused visa twice before. I have applied again for her B1/B2 visa. She has her interview on 27 of Feb. When applied it has given me DS-156, Interview letter etc but not DS-157. Does she need a DS-157 ?
Or else the system will not create DS-157 if the applicant is more than 65 years. I remember reading that a person who is applying for visa and have more than 65 years is not required to have DS-157. Any reply regarding this will be appreciated.
