givenname/surname swap in new passport ( urgent )

Hi:

i have this situation where my mother's passport has:

Old Passport : Surname: (x) Givenname: y
Visa in old passport : Surname: x Givenname: y
( visa valid till 2015 )
new passport : Surname: y Givenname: x

My mother visited us in 2005 and has her fingerprint and all
done. She is about to visit us in next few days and we noticed
this only now. Will this create problems with the immigration officer
in US?

Her new passport has refernce to old passport#and the visa
stamp in old passport has reference to the old passport#. Moreover
she has gone through fingerprinting last time she visited us. Will
this be an issue? Appreciate much the inputs.

I guess she has to carry her old passport along with the New Passport. You can staple them together. So if an question arrises she can show the Old Passport.
There are always problems with Names in Passport. So it shouldnt be a real big issue.

We have the same case now.
We (me and my wife) visited US in May 2012 with the visa in old passport as:
Given Name: X
Surname: Y
Now in new passport Given Name is : Y and Surname: X.
Similarly in my wife's case:
Old passport: Given Name: A B, SURNAME : BLANK
US VISA: Given Name: FNU, SURNAME: A B

New passport indicates: Given Name: A and Surname: B.
Will these be a problem for us to travel next month?
Any feedback please??
