DS 156 form modification

Hi,

My mother is applying for B2 visa.
Her first name is Mxxx
Fathers name-surname is Hxxx Dxxxx
Husbands name-surname is Kxxx-Lxxx


She has issued passport after marriage.
now in the passport her surname is Lxxx
But given name is Mxxx Hxxxx i.e. her father's name is attached to her name instead of the Husband's name.

But all details are exactly correct on the last page of the passport. (Last page consists of father's , mothers & husband's name) not the passport holder's name.

So does she require to change the given name on the passport to Mxxx Kxxx or let it remain what it has?


I took the appointment and now I am unable to change the 1 - 14 fields. Now if you suggest to change the name, can I make the changes in the DS 156 form in writing or should i cancel the appointment and reschedule it ?
Since cancellation will allow me to fill all details again.

make corrections on the d156

if u think it needs correction then when u print the d156 make the corrections
and the the vo know about it...i know q 1-14 cannot be changed by us and only way is to make corrections on the printed d156....plz don't reschedule ur interview....
