Clarification on Have you ever been refused a US Visa DS156?

First time my mother-in-law B2 visa was refused, because she went alone and she was not able to show family members proof of identiy like ration card (which says how many members are in family). At that time they didn't gave any form which says the reason why they refused.
So my question is do I need to say yes (Have you ever been refused a US Visa?) or no in DS156 Q31.
