Company Letter Format with leave granted pls help...

would like to invite my sister who is a Permanent Resident of Singapore and is working. Can someone please help me with the format of the company letter that she needs to take from her company saying that she is granted vacation for 2-3 weeks. My sister would apply as self sponsor. Is there any other documents that i need to send apart from the invitation letter?Please help as she has to apply for visa in a week at Singapore US Embassy.
