Few Questions on Visitors visa

I live in USA and try to bring my mom on visitors visa.

I have little medical problem where i need little help from my parents.

Please answer the below questions.

1) My name is A B C D in the passport where as in the Green Card it says A D. Which Name I should put in the I-134 form?

2)Me and my husband are going through the divorce in I-134 8th question what should I answer since my husband is paying me the child support, should i mention my son is wholly dependent or partially dependent?

3) I want my parents to come for tourist and also help when i get surgery, what should I put in the letter in the invitation and the letter to INS. Can I mention that I need my parents to come to USA for 2 purpose?

1. As per passport.
2. Yes, you can say your husband is paying the child support. Wholly dependent.
3. No. Just mention tourism.

For the 2nd question still I am not able to convience myself. I know you have answered the question but still want to confirm with you, even still my husband paying me the child support, i can still put my son as wholly dependent on me. There is no column asking me whether he is paying child support or not, the column says wholly dependent or partially dependent. After I say wholly dependent there is no row to write that I am getting child support from my husband. Please explain. Sorry about the confusion.

One more question - What are all the documents my father should take to HDFC? I know he should take the passport of his and my mother's but does he has to take a copy of my passport too. Please answer. After your email I will ask my father to go to the HDFC.

1. It does not really matter. This is an old form that was also used to sponsor people on green card. This question is not relevant anymore. But USCIS has not changed the form yet!
