Name Mismatch in DS-156

Hello,

On form DS-156, by mistake I entered my moms middle name also in the field of surname.

Correct : Surname: Siram
First and Middle: Udaya Durga

I Entered: Surname: Siram Udaya
First and Middle: Durge

How can I change this, Is it possible to change, will this cause any problem in their visa process.

u should fine. Ur mom can let consulate folks know
about the typo.

Q1-14 cannot be changed.

Also, I have seen atleast in Chennai, they look at the
passport and if visa issued, copy the same from
passport on to the visa papers (surname/givenname)

At the time of the interview your mother should inform the VO or the other officials about the mistake. Then it is corrected manually.
