Question in form I-134

Hello,

I am filling out the form I-134 to sponsor my father for a visitor visa. I am stuck at questions 9 and 10. I have sponsored my mother's visitor visa about 7 yrs ago. She got her visa approved and she has traveled to the States several times. For question 9, I don't remember the exact date when I submitted her affidavit. I can get the Visa issuance date from her Passport. What should I do?? Also, for question 10, since I sponsored my mother 7 yrs ago, does it mean that I submitted a Visa petition for her? As far as I remember, I just sent the supporting documentation and she filled in the forms in India. Unfortunately I don't have any of the original forms that I sent her and she doesn't have them either.

Any help is greatly appreciated in answering these questions.

You do remember that it was 9 years ago. Your mom's passport will have the issue date of the visa, which will give you an approximation on when you applied for the same.

Use that approximation and you'll do good.
