Help needed for filling I94 - for FNU

My mom got her visa and she will be travelling to US in Nov Mid.
She has her US visa with -

Firstname - FNU
GivenName - Thulasi Sundaram

How should the I 94 be filled? Should it be as per the US Visa?

I would appreciate your help.
