Mother's name different in her passport and mine.

I want to apply for my mother's visitor visa. But, the problem is that her name is different in her passport and mine. Actually, her first name was changed after she got married, but her passport has her old name since all her papers viz degrees, bank statements has her old name. But, in my passport, i wrote down the name that she got after she got married; since we all knew her by this new name.

How do i rectify this, since my husband will be sponsoring her visa and my passport copy will be required.Will providing an affidavit be enough?
