Dear Sir/Madam,
On my DS-156 form( generated with bar code ) Q.4 Issuance date is not the one as of my passport. How can I change it. I don't have any appointment date. What should I do now?

how can i Delete the application.

u cannot edit the first 14 questions in DS 156. Delete the application and refill and then schedule the appointment

Regarding changes in DS-156

You can not mofdify the information from box 1-14inthe online DS-156 form. If you need to change the information in the boxes 1-14, make corrections with the pen in the form DS-156 and take it to the consulate. Do not print another DS-156 form.
