Parents visitor's visa

Thanks in advance. Any input is greatly appreciated.

I am sponsoring my parents for B2-Visitors visa and I have a problem with my mother's name.

My Name in passport and Birth Certificate :
Siri Bolla (FirstName=Siri, LastName=Bolla)

Mother's Name in Passport:
Lakshmi Gannu (FirstName=Lakshmi, LastName=Gannu)

Mother's Name in my Birth Certificate:
Lakshmi Bolla (FirstName=Lakshmi, LastName=Bolla).

Since, the last name of my mother, in my mother's passport is not the same as mentioned in my birth certificate, I am not sure what to do.

All my mother's 10th class and degree certificates have Lakshmi Gannu (her maiden name).

Anyone faced the same situation. Is a simple affidavit for my mother enough?

That's no problem. Write her name as per passport in DS-156. Write new name as 'Other Names Used'.
