Hi,
I went to US on 9 July 2006 & return back to India on March 08, 2007. When I leave US the airlines person forget to tear off the I94 paper attached in my passport. On transit point (Frankfurt) I realized that & then I followed the steps mentioned in the below link:

I sent an application to ACS-USCIS, SBU, PO Box 7125 , London, KY 40742-7125, USA on March 10, 2007 along with the following documents:
1. Original I94 form of my passport
2. Original Boarding pass I used to depart the United States.
3. Photocopy of the entry stamp in my passport in India.
Now my concern is that How will I know whether the information is updated in USCIS database or not.
