How to get Visa Dates for Both Parents Together

Hi all,

How do I apply for a visitor's visa for both my parents together on the same date ?

While filling the forms for the visa are they to fill two separate forms or they can fill out ony one and take the appointment together?

Do they have to Pay Two Fees each to HDFC ?
