VFS online appointment

In order to take online appointment through VFS website, we need to fill DS 156, but on top of the application, the expiry date is 2007.
Is it ok to fill & use that form and proceed for appointment???
Do we have to fill separate form again which has not expired from other website???
OR, can we use just online appointment from VFS and fill form from other website???

Plz give details regarding this

No problem with exp date of DS 156 form

Yes, we used same visa form exp 2007 and no issues.
My Mom got visa yesterday.
