Missing Middle Name in DS156 - Pls. help

Hello,

While filling DS156 form for my mom, I forgot that she has middle name as per passport hence it got missed out.

Now, this field is not editable. Can u pls. help me ..what shd be done to address this issue.

Can I write middle name in printed form and ask my mom to initial it.
