Applying for Tourist Visa

Hello,

I have a question with respect to the passport application.

I am on H1 Visa in US. My wife is on H4 Visa. My father in law wants to apply for passport in India. The problem is, what name should he use as his First name and Last name in the Passport application form.

The reason I am asking this question is, we have 3 documents in which my father in laws names are different.
1) My Marriage Certificate : Suresh Chandra Sharma
2) My wifes Passport : Suresh Keshav Sharma
3) My wifes birth certificate : Suresh Sharma.


Now, he wants to apply for the passport, can you please let me know as to what name out of the above 3 should be used for passport application in India.

I would really appreciate if you can give me correct response.

Shouldn't matter ...

... make an affidavit certifying that all these names belong to the same person.

I think the best choice will be first name and last name only. Since the middle name differs, it will be best to omit it. As already advised, get an affidavit done.
