The visa was granted for 5 years but could not travel to USA. Will it be a problem while applying for renewal?

Should not be a problem. May have to explain why to the VO.

Depends on what your intended date of entry is. If you intend to enter before the existing visa expires, you need not as a visa is just for entry to the U.S. If you intend to enter after the visa expires, you of course need one before you enter. The thing is a visa should be valid at the time of entry to the U.S.
I am not sure what you mean by renewal. It's a new visa application.Check out H1 FAQs first!
