Visiting the U.S.

My fiance is from Ukraine, I would like for her to visit the United States during the summer months as she is a teacher and has her summers free. She has recently acquired her passport; my question is what else would she need to obtain to stay for approximately 5 weeks, if anything? Do you know of what is needed, again, if anything, to leave Ukraine? I do not know what their policy is concerning traveling to America, i.e., special permission or documents that must be filled out for permission to leave Ukraine.
