Can someone please tell me if its okay to fill "N/A" for Q8-( Other surnames used )and Q9-(Other first and middle names used) in DS156 form, or whether we need to fill something ? For example my sister's name is "Sri Devi" and she is usually called as "Devi" which sounds short. However in all other official records her name is "Sri Devi". So would "Devi" be considered as other surname/first/middle name? Please suggest. I would want to just fill Q8 and Q9 as "N/A" . Please let me know if that should be fine or does it really matter as i remember i had read somewhere that its better to fill atleast first/middl name or alias instead of filling it as "N/A" Please confirm
