I appreciate if you could clarify the below questions:
(1) Could I fill in the VFS application form for my parents?
(2) Is this form the same as DS-156? If so, then I need to mention my name on line 39 and 40 of DS-156. How do I sign the form if I fill electronically.
(3) Could I fill both mother and father forms at the same time on VFS site.
(4) Do I need to choose the date before I fill the form? Or could I select the interview date at a later time.
(5) I read on US consulate page that Surname and First need to be explicitly mentioned in the passport. Not as a single line. Is this a requirement for Visitor visa applicants?
