Visa application and some other help needed

I wanted to know where can i fill up the form for a "Visitor visa or a Tourist visa" which form?
2)i want to go to usa for 15 days to meet my girlfriend
3)my dad is going to sponsor my trip
4)i work for a company + i help my dad in his business
5)Should i ask my girlfriend to send me an invite letter or so..and if yes then please let me know wat all documents does she need to send
6)i'm going to visit my friend(male) too who is in texas,do i ask him to send me an invitation letter
7)it is just a 15 days trip..1 week with my girlfriend and 1 week with my friend
8)what do u think will be the best??
9)i've been in a relation with her for about 5 mnths now and plan to spend newyear's with her,sad part is we do not have nay pictures with each-other
10)please help it is required,i have a fresh passport
11)i know it is a very weak case
12)but please give me the best of suggestions which is very much required
13)my age is main factor.i'm just 21 going to turn 22 in january

Yes i did meet her online but we have met in person to when she had come down to india..
