advice for cs exam visa

hi,i once failed in usmle cs exam in 2008.i previously got single entry visa.i returned within the time limit.when i applied visa next time fir cs,i was rejected saying that i failed cs.what can i say next time to improve my chances? will i get visa later? or i should forget usmle?advice plz...

actually amrican accent was difficult for me,thus i failed first time.but this time i prepared well and practised with online friends and dvd's and now i am confident.how can i convince the visa officer?
