DS-156 expired on 8/31/2004?

Hi all,
When I started filling form DS-156, I noticed that at the top of document it says 'expires 8/31/2004'. I tried finding on state govt site, google but couldnot find a DS-0156 form which has expiry date greater than 8/31/2004. What to do?

My folks are planning for interview on last week on Nov and I started getting papers ready. Should we go ahead with this form or wait for sometime?
