Is it true that the ds-156 application has to be back-back single page and not 2 pages ????

Did you pick up the forms from TT services or did you photocopy them this way after downloading the document

How serious is this issue ????

You have to have DS-156 back to back.

Easiest would be just to either print back to back or copy them back to back.

You can also pick the form up at TTS.
