    Hi,

    I have few questions abt form DS-156

    1) In Section 22, do we need to write any date? We did not book any ticket yet. So do we need to write tentative date, we are planning to visit, or should write "NA"?

    2) In section 28 "Who Will Pay For Your Trip?" Should we write just " My daugher" or we should mention name too?

    3) In section 29, 30, 31, after selection "No", do we still have to write "NA" in questions When, how long etc? or we can leave that blank?

    Also,
    1) As we are suppose to send docs 20 days in advance to consulate before interview date. What all documents are reqd to be send?

    2) Is father/mother's affidavite (reg. property etc) also need to be send?

    3) Do we need to send original passport?

    4) what all original docs should be sent?

    1. Write your tentative date, or something like 'First week of June' or 'Month of July' etc.

    2. Daughter, and then write name too.

    3. Write N/A.

    2. Don't send the documents. Show them at the time of interview.

    3. No. Just photocopies.

      Thanks for your help. Few more doubts,

      1) Since it is written on site (www.visa-services.com/int...line.html) that "Please report not earlier than 15 minutes prior to the appointment time. Remember to carry your passport, Appointment Letter and Fee Receipt!" I was assuming that we don't have to send applicant's original passport by courier and instead have to carry when we go for interview.

      2) Does applicant need to send sponsor's Original I-134 by courier or just have to carry during interview?

        In addition to above 2 questions, I have 2 more questions

        1) Is it required to send sponsor's birth certificate by courier (before appointment) or it's reqd just to carry it during interview?

        2) There is an additional photo requited to be sent with DS-156. How should we send that? DO we need to attach/staple that with form 156 or just put it in the envelop with other documents?

          1. You are right. Don't send original. Just photocopy of passport.
