query reg. i-134 form

Hi,

1) Should I take back to back printout of form I-134 or single side?

2) My mother as well as father are applying for visitor visa. I am sending 2 separate I-134 for them. Do I need to write in form (that I am sending for my father) my mothers details in section #3 "Name of spouse and children accompanying or following to join person:" and vice versa?

3) If there is nothing to write in few sections for example in section #3, in child area, i have nothing to write, should I leave that as blank? Or I need to write something (NA or None)? if yes, then whether none or NA?
