  • Queries regarding the DS-156 and DS-157 forms

Hello there, I have read the FAQ section here, but could not find the answers to my queries, so, asking here.

I am an Indian citizen (from Punjab) applying for a non-immigrant US visa (B2). I have the following queries regarding DS-156 and DS-157 forms:

    1. Do I have to write "NA" or "nil" when I do not have to fill up anything/ when I am not eligible to fill up anything?

    2. Do I need to sign the DS-156 (where it asks to sign) after printing it out?

    3. Can I use inkjet printer in "best dpi" (in my case HP officejet 4300 series)?

    4. What is the visa "issuance fee"..i.e. the fee I need to pay when I am issued a visa?

    5. What should I write in DS-157 form box no. 3 where I am asked to write my full name in my native alphabet? I am from Punjab, so, should I write my name in Punjabi? And, should I write this name after printing the form out or online using the native fonts?

    Thanks!
