Help in Filling I-134 Form

Here is the situation:

I entered US very first time on 3/11/2003. I stayed for 3 years and then I went to India in April 2006. I came back to US on 7/7/2007. Again I went for vacation on 10/15/2008 and returned to US on 11/17/2008.


Now, for the I-134 form, which date should I enter.

Is it 3/11/2003 OR 7/7/2007 OR 11/17/2007?

The question in the form is: I am _______ years of age and have resided in Unites States since (date)____________________.


My current status is L1 A.

The question is, what is ur present status in us? Are you citizen or are you GC holder? (actually it dosn't matter) U have to put the issue date of ur FIRST green card. (printed on ur green card)
